Forms, surveys and quizzes
Change how your forms look
Pick a theme, add your logo and photos, and style each question. It works the same for forms, surveys and quizzes.

Every form, survey and quiz has a Design tab. Pick a theme and you're nearly done: it sets the colours, the type, the layout and how the answers look.
Pick a theme
- Open Forms and choose Edit beside your form.
- Choose the Design tab.
- Under Theme, pick one. The preview changes straight away.
- Choose Save changes. People see the new look once it's saved.
A theme changes how the form is built, not just its colours:
| Theme | What you get |
|---|---|
| Your colours | Your account colour on a clean white card |
| Paper | A printed page, with serif type and ruled lines |
| Midnight | Dark and full screen, one question at a time |
| Workshop | Loud and square, for trades, gyms and anything that means business |
| Bloom | Soft and centred, with pill answers |
| Meadow | Warm greens beside a picture or a colour panel |
| Monochrome | Black on white, with big type and numbered answers |
| Citrus | Bright and rounded, with answers in a grid |
| Photo backdrop | Your own photo behind a neat card |
Choose a layout
Under Layout, pick Card, One at a time or Split with image. With One at a time, people see one question on the screen and move on when they've answered it.
Surveys, quizzes and anything shown one at a time also have a Progress setting: Bar, Dots, Steps, Percent or None.
Add your logo and photos
Under Logo and cover:
- Logo replaces your account's initials and name.
- Cover image is a wide banner above the form.
Split with image has its own Image beside the form. On a phone it sits above the form, and embedded forms leave it out. The Photo backdrop theme asks for a Background photo.
Change the details
Open Customise this theme to change the Colours, Background, Typefaces, Corners, Answer boxes, Button and Button shape. Undo these changes takes you back to the theme as it came.
Choosing another theme clears these changes. Your logo, photos and words stay.
Style one question
Most questions look right with whatever the theme gives them. To change one, go to the Questions tab and open Style and help text on that question.
- Help text is a short line under the question, and you can add an Icon beside the question.
- Show the answers as turns a pick-one question into a Toggle, Numbers, a Slider, Stars, Hearts or Faces.
- Show the rating as does the same for a rating question.
- Beside each answer adds a tick, letters, numbers, an icon or an emoji.
- Lay the answers out as gives you columns, Pills, Large cards or Image cards.
To give one answer its own icon, emoji, image or short description, use the button beside it in the list of answers. Image cards show the image.
Welcome and thank-you screens
Surveys and quizzes can open with a screen of their own. Under Welcome screen, tick Show a welcome screen first, then add a Heading, a line or two, a Button, and an Icon or Image.
Open Thank-you screen to set what people see at the end, with a Heading, an Icon or Image, and a Button that links anywhere, like your booking page. The message itself is under After sending.
Next, share your form, or run a survey or quiz.
